Job Summary

We are seeking a highly skilled and compassionate LTSS Service Coordinator to join our team at Elevance Health. As a key member of our care coordination team, you will play a vital role in ensuring that individuals with specialized needs receive the support and services they require to live independently and thrive.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans that address the unique needs and goals of each individual.
  • Collaborate with interdisciplinary teams, including medical directors, to ensure seamless care coordination and effective communication.
  • Engage with individuals, their families, and caregivers to understand their needs and preferences, and to provide education and support to promote self-advocacy and empowerment.
  • Identify and address gaps in care, and coordinate with healthcare providers and community resources to ensure timely and effective interventions.
  • Manage non-clinical needs, including transportation, housing, and employment, to support individuals in achieving their goals and maintaining their independence.
  • Document and track progress, and make recommendations for service adjustments as needed.
  • Participate in quality improvement initiatives and contribute to the development of best practices in care coordination.
Requirements
  • Bachelor's degree in a healthcare-related field, or equivalent experience.
  • Minimum of 2 years of experience working in a social work agency or similar setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Ability to work independently and as part of a team, with a high level of organization and attention to detail.
  • Knowledge of relevant laws, regulations, and policies governing long-term services and supports.
Preferred Qualifications
  • Master's degree in a healthcare-related field.
  • Experience working with individuals with complex needs, including those with intellectual and developmental disabilities.
  • Knowledge of person-centered planning and care coordination principles.
