SUMMARY:

The Care Management Team Leader is tasked with overseeing patient care responsibilities as a care navigator while also managing and supporting the service delivery within the care management division. This role includes supervising care navigators and other relevant personnel as necessary. The Team Leader engages in quality assurance initiatives and coordinates services for participants, ensuring the smooth operation of daily activities across various sites and programs.

RESPONSIBILITIES:

Provide orientation, training, and ongoing support to designated care navigation staff, ensuring compliance with departmental documentation and service delivery standards. Participate in the recruitment and performance evaluation of team members.

Develop and implement training materials for both new and experienced Care Navigators, aimed at enhancing service quality for patients, in collaboration with the program associate and care management coordinator.

Conduct administrative supervision sessions with care navigators to offer constructive feedback and support, guided by the program supervisor.

Execute quality assurance tasks under the direction of the care management coordinator.

Facilitate weekly team meetings and huddles, and represent the team in internal and external committees as required.

Assist in the daily management of staff assignments and responsibilities to ensure effective office and program operations.

Support Directors in compiling program reports, particularly concerning the attainment of performance objectives and targets.

Participate in regional, statewide, and internal meetings, along with other relevant gatherings as needed.

QUALIFICATIONS:

A Bachelor’s degree in social work, human services, psychology, nursing, or a related field is preferred.

Proficiency in computer applications, including MS Word, Excel, and PowerPoint.

Exhibit strong organizational, interpersonal, and communication skills, both oral and written, with the ability to manage multiple tasks simultaneously.

Capability to generate, interpret, and analyze data from various sources.

Prior experience in a care management role or a similar position is preferred.

Supervisory experience of at least one year is preferred.
