POSITION TITLE: Case Manager

DEPARTMENT: Women of Hope - Vine

PROGRAM: Housing and Homeless Services

KEY RESPONSIBILITIES:

The Case Manager will deliver comprehensive case management services to residents at Women of Hope-Vine. This role ensures the protection of clients' confidentiality and adheres to professional standards of care. The Case Manager will be well-versed in protocols to fulfill all contractual obligations with relevant health departments and ensure compliance with regulations for Licensed Personal Care Homes.

The primary focus is to provide direct support addressing the mental, medical, physical, and emotional needs of residents, guiding them towards achieving independence. Collaboration with interdisciplinary teams and family members is essential to advocate for and support each resident effectively.

ESSENTIAL DUTIES:

  • Conduct thorough intake assessments, service planning, and discharge planning for each resident.
  • Assist residents during their transition into the facility, managing orientation and curfew schedules.
  • Oversee medication management, including logging medications, maintaining monthly records, and providing education on medication use.
  • Schedule regular meetings with residents to monitor progress and establish achievable goals.
  • Coordinate with healthcare providers to ensure residents receive necessary physical and mental health services.
  • Maintain detailed and accurate records of resident progress and medical information.
  • Support residents in exploring and applying for relevant entitlements.
  • Provide assistance with Activities of Daily Living (ADL) and personal care, including financial management.
  • Research and recommend programs that prepare residents for permanent housing.
  • Engage in weekly meetings with the social work supervisor to discuss resident progress and share insights.
  • Ensure effective communication with housekeeping, maintenance, and kitchen staff regarding resident needs.
  • Participate in on-call responsibilities with the case management team.
  • Demonstrate flexibility by assisting in various areas as needed, including recreational activities and holiday planning.
  • Complete annual training related to agency policies and procedures.
  • Attend staff meetings and utilize in-service training opportunities.
  • Assist the social work supervisor and program director with additional tasks as required.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Understanding of the agency's mission and philosophy.
  • Bachelor's degree in social work or a related field.
  • Strong organizational skills with the ability to manage multiple tasks and respond effectively to crises.
  • Commitment to working with women facing severe mental health challenges and experiencing homelessness.
  • Ability to work collaboratively as part of a team while maintaining professionalism and respect.
  • Certification in CPR and First Aid is required within the first month of employment.
  • Completion of the Direct Care Staff Person Training Course within the first month of employment.
  • Completion of state medication administration training along with agency-specific training within the first month of employment.
  • Possession of a valid state driver's license.
