POSITION TITLE: Case Manager

DEPARTMENT: Women of Hope - Vine

PROGRAM: Housing and Homeless Services

KEY RESPONSIBILITIES:

The Case Manager will deliver comprehensive case management services to residents at Women of Hope-Vine. This role is essential in safeguarding the confidentiality and rights of clients while adhering to professional standards. A thorough understanding of the protocols is required to ensure compliance with all contractual obligations with relevant health and human services departments.

The Case Manager will:

  • Conduct detailed intake assessments, service planning, and discharge planning for each resident.
  • Assist residents in their transition into the facility, managing orientation and curfew procedures.
  • Oversee medication management, including logging incoming medications, maintaining monthly medication records, and providing education on medication usage.
  • Engage regularly with residents to monitor their progress towards established goals, collaborating with interdisciplinary teams as necessary.
  • Ensure attendance at required physical and mental health services, maintaining compliance with state regulations.
  • Keep accurate and updated records of resident progress and medical notes.
  • Support residents in exploring and applying for relevant entitlements.
  • Provide assistance with Activities of Daily Living (ADL) and personal care services, including financial management.
  • Research and recommend programs that facilitate job training and social rehabilitation in preparation for permanent housing.
  • Participate in weekly meetings with the social work supervisor to discuss resident progress and share insights.
  • Maintain open lines of communication with all staff members involved in resident care.
  • Share on-call responsibilities with the case management team.
  • Contribute to team efforts by assisting in various capacities as needed.
  • Engage in recreational activities and assist in planning events for residents.
  • Complete required training on relevant policies and participate in staff meetings.
  • Assist supervisors in tasks that promote the welfare of residents and the overall program.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Understanding of the agency's mission and philosophy.
  • Bachelor's degree in social work or a related field.
  • Strong organizational abilities and crisis management skills.
  • Commitment to working with women facing severe mental health challenges and homelessness.
  • Ability to work collaboratively within a team, demonstrating respect and professionalism.
  • Certification in CPR and First Aid within the first month of employment.
  • Completion of required training courses within the first month of employment.
  • Valid state driver's license.
