Position Overview:
The Registered Nurse Case Coordinator employs advanced nursing expertise to effectively manage care for a designated patient demographic, ensuring seamless navigation through the healthcare continuum.
Responsible for orchestrating all elements of discharge planning, the RN establishes a secure discharge strategy for assigned patients.Engages with colleagues, visitors, and other personnel in alignment with the core values of Thomas Jefferson University.
Acts as an advocate for patients and their families throughout the entirety of their care journey. Serves as a vital resource to enhance communication among the healthcare team and the patient/family. Facilitates patient advancement during their hospital stay. Analyzes patient length of stay and barriers data to promote patient progression during Multidisciplinary Rounds and Escalation/Outlier meetings. Monitors patient status daily to pinpoint issues necessitating immediate intervention or attention that may affect the established discharge plan.
May lead daily patient Plan of Care Huddle Rounds, acting as the Patient Progression Leader or providing support to the designated Patient Progression Leader.
Oversees patient progress, intervening as needed to ensure efficient patient advancement. Proactively identifies and addresses obstacles to patient care and/or discharge progression. Elevates unresolved challenges to Department Leadership.Educational Requirements:
Bachelor's degree in Nursing is mandatory AND Experience: A minimum of 3 years in case management or discharge planning is essential. Please specify the minimum educational qualifications and experience for this role. Indicate whether they are required or preferred.
Licensure and Certification:
CCM certification is preferred; an active and relevant Registered Nurse license in Pennsylvania/New Jersey is required. Covid Vaccination is a prerequisite for employment at Thomas Jefferson University for individuals working within clinical settings.
If not currently vaccinated, candidates must receive the vaccination prior to the hire date if offered employment, unless an approved medical or religious exemption is obtained.
