REPORTS TO: Program Director
DIRECT REPORTS: Case Management Team
Key ResponsibilitiesThe Women of Hope program serves as a residential Safe Haven, accommodating up to 16 young women aged 18-24 who have faced homelessness and mental health challenges.
The Social Work Supervisor is tasked with overseeing a dedicated case management team, providing essential guidance and support. This role is crucial in assisting the Program Director to ensure the program operates effectively and efficiently, guaranteeing that all residents receive appropriate support and that services comply with relevant regulations.
Main Duties- Conduct weekly supervision sessions with the case management team.
- Identify and document training requirements for team members.
- Perform annual performance evaluations for the case management team.
- Assist the team in maintaining up-to-date housing plans for residents.
- Review case records and medication tracking for accuracy.
- Be available for on-call medication-related issues.
- Evaluate case notes weekly, ensuring proper documentation using the D.A.P. format.
- Monitor daily logs to verify adherence to procedures and accuracy of reports.
- Oversee the admission and discharge processes for residents.
- Submit necessary event forms as required.
- Maintain a manageable caseload of three residents.
- Engage with residents to foster positive relationships.
- Participate in monthly meetings with residents.
- Support lunch services for the Mercy Café Day program.
- Assist with case management needs related to the Mercy Café Day program.
- Meet weekly with the Program Director for updates and coordination.
- Attend concurrent reviews with program analysts.
- Conduct quarterly reviews of medication distribution procedures.
- Assist in preparing for audits and inspections of programs and records.
- Ensure compliance with agency, city, and funder requirements.
- Engage in monthly staff meetings and required training sessions.
- Collaborate with community organizations to enhance program visibility and services.
- Perform additional tasks as assigned.
- Commitment to the agency's values and mission.
- Minimum of a Master of Social Work (MSW) or related degree.
- Demonstrated leadership capabilities.
- Experience working with women facing homelessness, trauma, and mental health issues.
- Certification in CPR and First Aid.
- Medication administration certification.
- Ability to establish professional partnerships with community organizations.
- Strong written and verbal communication skills.
- Valid driver's license preferred.
