Job Summary:

AW Health Care is seeking a skilled Field Care Coordinator to join our team. As a CNA or experienced Caregiver with one year or more of professional work experience, you will be responsible for assisting with training and supervision of caregivers, client/employee advocacy, and related care management activities to ensure overall satisfaction of an assigned group of clients and caregivers within the Home Care Department (Private Duty and Medicaid).

Responsibilities:
  • Evaluate and train staff in the field to ensure competency of the employees for Private Duty and Medicaid department;
  • Assist with in-house Caregiver training, including General Orientation, Skills Lab, Annual In-Services, On-the-Job Training, as needed;
  • On-going education, supervision, and training of the Caregivers in the field;
  • Identify high risk factors, barriers to goals, and strengths that will assist the client and caregivers in achieving goals;
  • Assist with the assignment and scheduling of field staff to provide care and services for Private Duty and Medicaid clients;
  • Assure quality of services by gathering information from direct observation, telephone, or in writing, all resources involved in the client plan of care;
  • Attend case management meetings as scheduled;
  • Act as a positive role model for the company when dealing with employees and the public;
  • Communicate to Supervisor any conflicts or difficulties with employee assignment;
  • Communicate to Supervisor or Case Management RN any changes in the client's condition;
  • Answer all pages, telephone calls or communications promptly and courteously;
  • Rotate on-call responsibilities with other members of management;
  • Work with all members of the private pay team and across all branches to grow private pay revenue and profit;
  • Support for on-call staff, evenings and weekends, as needed;
  • Cover shifts, in the event of staffing shortage, with direction of management;
  • Assist with office-related tasks, as well as ensuring accurate opening packets are available with an adequate supply for each week;
  • Maintain confidentiality of all information in accordance with state/federal regulations, company policy and HIPPA requirements;
  • Complete all required documentation and turn it in to the office as directed;
  • Follow all policies and procedures, as they are defined in the Employee Handbooks.
Working Environment:

Works within the agency offices and patient homes, and will be required to travel between offices and client home. There is mileage reimbursement for client transportation/errands and travel in-between clients.
