Position Overview:
The Field Care Coordinator at AW Health Care is a dedicated full-time professional responsible for overseeing the training and supervision of Certified Nursing Assistants (CNAs) and Caregivers. This role involves managing schedules, providing backup support, and covering shifts as necessary. The ideal candidate will possess strong administrative capabilities and experience in care management to ensure the satisfaction of clients and caregivers within the Home Care Department.


Work Schedule:
Monday - Friday, 8:00 AM - 5:00 PM, with flexibility.

Key Responsibilities:
• Evaluate and train field staff to ensure their competency in providing care for Private Duty and Medicaid clients.
• Assist in the training of Caregivers through various programs, including General Orientation and Skills Lab.
• Provide ongoing education and supervision to Caregivers in the field.
• Identify high-risk factors and barriers to client goals, leveraging strengths to facilitate success.
• Assist in the assignment and scheduling of field staff for client care.
• Ensure quality of services through direct observation and communication with involved resources.
• Participate in scheduled case management meetings.
• Serve as a positive role model in interactions with employees and the public.
• Communicate any conflicts or changes in client conditions to supervisors promptly.
• Manage on-call responsibilities alongside other management team members.
• Collaborate with the private pay team across various branches to enhance revenue and profitability.
• Provide support for on-call staff during evenings and weekends as needed.
• Cover shifts during staffing shortages under management direction.
• Maintain accurate documentation and filing for all clients while ensuring confidentiality.


Work Environment:
This position requires travel between agency offices, patient homes, and independent living communities.

Qualifications:
• Minimum age of 18 years.
• Valid Driver's License and reliable transportation with full coverage auto insurance.
• CNA certification preferred but not mandatory.
• Strong administrative skills and experience in scheduling and training.
• At least two years of paid experience as a CNA or Caregiver.
• Ability to work with clients who have pets or smoke.
• Flexibility to adapt to changing priorities throughout the day.
• Experience in caring for clients with behavioral or mental health needs is essential.
• Understanding of growth and development principles across the lifespan is crucial for effective care delivery.

Physical Requirements:
• Ability to engage in physical activities as needed.
• Capacity to stand and be active for extended periods.
• Moderate lifting and extensive bending may be required.
