Home Care Field Coordinator

Position Overview:
The Field Care Coordinator at AW Health Care is a dedicated full-time professional responsible for overseeing the training and supervision of Certified Nursing Assistants (CNAs) and Caregivers. This role includes managing client assignments, providing backup for scheduling, and covering shifts as necessary. The ideal candidate will possess strong administrative capabilities and experience in care management to ensure the satisfaction of clients and caregivers within the Home Care Department.
Work Schedule:
Monday - Friday, 8:00 AM - 5:00 PM, with flexibility.

Key Responsibilities:
• Evaluate and train field staff to ensure competency in Private Duty and Medicaid services.
• Assist with in-house training for Caregivers, including orientation and skills development.
• Provide ongoing education and supervision for Caregivers in the field.
• Identify high-risk factors and strengths to support clients and caregivers in achieving their goals.
• Assist in scheduling and assigning field staff for client care.
• Ensure quality service delivery through direct observation and communication with involved resources.
• Participate in case management meetings as required.
• Serve as a positive role model for the organization.
• Communicate effectively with supervisors regarding employee assignments and client condition changes.
• Respond promptly to communications and support on-call staff as needed.
• Maintain confidentiality and comply with all regulations and company policies.
Work Environment:
This position involves working in agency offices, client homes, and independent living communities, requiring travel between locations.
Qualifications:
• Minimum age of 18 years.
• Valid Driver's License and reliable transportation with full coverage insurance.
• CNA certification preferred but not mandatory.
• Proficient in reading, writing, and documentation.
• Strong administrative background with some scheduling experience preferred.
• At least two years of paid experience as a CNA or Caregiver.
• Ability to work with clients who have pets or smoke.
• Flexibility in scheduling and adaptability to changing priorities.
• Experience with behavioral or mentally disabled clients is essential.
• Knowledge of growth and development principles across the lifespan is crucial for effective care delivery.
Physical Requirements:
• Ability to engage in physical activities as needed.
• Capability to stand and be active for extended periods.
• Moderate lifting and extensive bending may be required.
