Position Overview:
The Field Care Coordinator at AW Health Care is a full-time role responsible for overseeing the training and supervision of Certified Nursing Assistants (CNAs) and Caregivers. This position includes conducting orientations, providing scheduling support, and covering shifts as needed. The ideal candidate will possess strong organizational skills and the expertise to manage care activities effectively, ensuring the satisfaction of both clients and caregivers.


Work Schedule:
Monday - Friday, 8:00 AM - 5:00 PM, with flexibility.

Key Responsibilities:
- Evaluate and train field staff to ensure they meet competency standards for Private Duty and Medicaid services.
- Assist in the training of Caregivers through various educational programs and on-the-job training.
- Provide ongoing supervision and support to Caregivers in the field.
- Identify and address high-risk factors and barriers to achieving client goals.
- Manage the assignment and scheduling of field staff to ensure adequate coverage for clients.
- Ensure quality service delivery by gathering feedback from observations and communications.
- Participate in case management meetings as required.
- Serve as a positive representative of the company in all interactions.
- Communicate effectively with supervisors regarding staff assignments and client conditions.
- Respond promptly to all communications.
- Share on-call responsibilities with management team members.
- Collaborate with the private pay team across various locations to enhance revenue and service delivery.
- Provide support for on-call staff during evenings and weekends as necessary.
- Assist with administrative tasks and maintain accurate client documentation.
- Uphold confidentiality in accordance with regulations and company policies.
- Complete all required documentation accurately and timely.


Work Environment:
This role involves working in both office settings and client homes, requiring travel between locations.

Qualifications:
- Minimum age of 18 years.
- Valid Driver's License and reliable transportation with full coverage insurance.
- CNA certification preferred but not mandatory.
- Strong written and verbal communication skills.
- Background in administrative tasks and scheduling is advantageous.
- At least two years of paid experience as a CNA or Caregiver.
- Ability to work with clients who have pets or smoke.
- Flexibility in scheduling and adaptability to changing priorities.
- Experience in caring for clients with behavioral or mental health needs is essential.
- A comprehensive understanding of developmental principles across the lifespan is crucial for this role.

Physical Requirements:
- Ability to engage in physical activities as needed.
- Capability to stand and be active for extended periods.
- Moderate lifting and extensive bending may be required.
