ROLE OVERVIEW

The Housing Stability Case Coordinator will play a vital role in supporting individuals who have experienced homelessness by facilitating their access to and maintenance of permanent housing. This position involves working closely with program participants to establish and achieve personal goals, meet essential needs, and enhance their self-sufficiency, all aimed at ensuring their housing stability and overall well-being during their transition.

KEY RESPONSIBILITIES

  • Establish rapport with a caseload of up to 25 participants, including families and individuals.
  • Foster strong relationships with landlords and property managers.
  • Assist participants in navigating housing or voucher programs to secure suitable housing options.
  • Collaborate with participants to create and monitor an Individualized Service Plan (ISP) tailored to their strengths and goals.
  • Guide participants through the completion and submission of necessary housing and benefit applications, including document management.
  • Connect participants with additional services and benefits available to them.
  • Educate participants on tenant and landlord rights and responsibilities.
  • Provide coaching on strategies to maintain housing stability.
  • Identify and address potential barriers to housing stability, such as late payments or lease violations.
  • Assist participants in accessing basic needs and developing skills for daily living.
  • Support participants in finding and maintaining community connections.
  • Complete required documentation, including case notes and housing stabilization plans, within specified timeframes.
  • Maintain participant records in compliance with all relevant regulations and standards.

QUALIFICATIONS
  • Bachelor's degree in social work, psychology, sociology, counseling, or a related field, or relevant certification/licensure, or a high school diploma with significant experience in social services.
  • A minimum of one year of experience in case management.
ADDITIONAL SKILLS
  • Experience with Housing the Homeless (HTH) database and proficiency in software applications such as Google Workspace.
  • Strong communication skills, both written and verbal.
  • Understanding of structural racism and its impact on communities.
  • Commitment to ending homelessness and centering the voices of those with lived experience.
