Position Overview

Under minimal supervision, the Patient Safety Specialist is responsible for evaluating patient safety incidents and collaborating with hospital management, clinical teams, and the Risk Management Department to foster organizational transformation aimed at achieving high reliability, eliminating harm, and promoting a Just Culture.

This role is essential to the patient safety initiative, serving as a key resource for departmental leaders, physician colleagues, unit dyads, and frontline staff.

Works closely with leadership to enhance and prioritize the ongoing commitment to a culture centered on patient safety.


Key Responsibilities

The following responsibilities outline the general nature and level of work expected from individuals in this role. They are not intended to be an exhaustive list of all duties performed.

  • Facilitates activities aimed at progressing towards a High Reliability Organization (HRO) with a focus on zero harm.
  • Identifies, measures, monitors, and assesses safety improvement initiatives within the organization, aligned with a Just Culture framework.
  • Investigates patient safety incidents, trends, and sentinel events.
  • Collaborates with the Patient Safety Manager and senior leadership to guide the safety program.

Specific Duties Include:
  • Overseeing the Root Cause Analysis (RCA) process, which entails conducting investigations, leading RCA meetings, and performing apparent cause analyses (ACA) as necessary.
  • Formulating action plans in compliance with The Office of Healthcare Quality (OHCQ) and Joint Commission (TJC) regulations for sentinel and adverse event reviews, reporting, and improvement strategies.
  • Facilitating the creation, execution, and monitoring of action plans.
  • Conducting chart reviews relevant to the discipline/program and preparing summaries/timelines of patient safety events for informed discussions and problem-solving.
  • Enhancing and nurturing collaborative relationships with key medical staff and providing guidance to clinical and operational leadership to ensure monitoring of key patient safety metrics and alignment with departmental goals.
  • Partnering with leaders in safety, quality, performance improvement, and data management to optimize data utilization for enhancing Patient Safety and minimizing preventable harm.
  • Contributing to the patient safety team to identify and lead initiatives aimed at improving event reporting and responses to near-miss incidents.
  • Ensuring adherence to external regulatory and accrediting bodies (e.g., The Joint Commission, CMS, Maryland State Department of Health Office of Healthcare Quality).
  • Ensuring that Medical Center and departmental policies, procedures, and standards comply with regulatory and accrediting agency requirements related to patient safety.
  • Collecting and analyzing data, developing presentations, providing consultation, and leading service-specific and hospital-wide committees.
  • Creating and implementing educational programs for staff and medical personnel concerning patient safety, prevention of medical errors, and compliance with regulatory standards.
  • Participating in staff orientation and contributing to organizational safety communications, alerts, and recognition programs.
  • Serving as a consultant and resource for unit managers and clinical representatives, assisting with the development of clinical and safety indicators and process enhancements.
  • Managing aspects of the Event Reporting System, including reviewing daily reports, notifying managers for follow-up, and tracking trends to mitigate preventable harm.
  • Participating in proactive risk assessments and assisting with the implementation of safety culture assessments as directed.
  • Leading or participating in key hospital committees and teams to ensure that safety and the prevention of medical errors are prioritized.
  • Educating leaders and clinical staff on fostering a supportive environment that balances a non-punitive response to human error with appropriate accountability for reckless behavior.
