Position Summary

Under limited supervision, the Patient Safety Specialist is responsible for analyzing patient safety incidents and collaborating with hospital leadership, clinical teams, and the Risk Management Department to promote a culture of safety and high reliability. This role is essential in supporting the patient safety program and working closely with departmental leaders, physicians, and frontline staff.

Key Responsibilities

  1. Assist in coordinating activities aimed at achieving High Reliability Organization (HRO) status with a focus on eliminating harm. This includes identifying, measuring, and evaluating safety improvement initiatives.
  2. Investigate patient safety incidents, trends, and sentinel events to enhance safety protocols.
  3. Collaborate with the Patient Safety Manager and senior leadership to oversee the safety program, including:
  • Managing the Root Cause Analysis (RCA) process, conducting investigations, and facilitating RCA meetings.
  • Developing action plans in compliance with healthcare quality regulations for event reviews and improvement strategies.
Conduct thorough investigations of adverse events, including chart reviews and preparation of safety event summaries for collaborative discussions.Foster collaborative relationships with medical staff and provide guidance to ensure patient safety metrics are monitored effectively.Work with safety, quality, and performance improvement leaders to utilize data for enhancing patient safety and reducing preventable harm.Lead initiatives to improve event reporting and responses to near-miss incidents, managing patient safety projects identified through reports and data analysis.Ensure compliance with regulatory agencies and accrediting bodies, maintaining adherence to safety standards.Collect and analyze data, develop presentations, and lead committees focused on patient safety initiatives.Design and implement educational programs for staff regarding patient safety and regulatory compliance.Act as a resource for unit managers and clinical representatives, assisting in the development of safety indicators and process improvements.Oversee the Event Reporting System, including reviewing reports, identifying trends, and tracking events to minimize harm.Participate in proactive risk assessments and assist with safety culture assessments as needed.Engage in key hospital committees to address safety and error prevention strategies.Educate staff on fostering a supportive environment that balances accountability with a non-punitive approach to human error.

Qualifications

  1. Bachelor's degree in Nursing or a related healthcare field; a Master's degree is preferred.
  2. Current licensure in a relevant field is required.
  3. A minimum of three years of experience in patient safety within an acute care setting is essential.
