POSITION SUMMARY:
The Mobile Clinical Services Coordinator is responsible for delivering community-focused therapeutic interventions for individuals facing mental health challenges or co-occurring disorders. This role emphasizes community-based mental health treatment utilizing evidence-based, trauma-informed, and individualized approaches.
QUALIFICATIONS:
- A Master's degree in social work, counseling, or a related discipline, with a preferred license to practice (e.g., LMSW, LCSW, LMHC, LMFT). Candidates with limited permits (e.g., MHC-LP, MFT-LP) may be considered, but full licensure must be achieved within 1-2 years of joining the organization.
- Demonstrated competency in substance abuse and/or mental health counseling, supported by at least one year of relevant training and/or experience in assessment and treatment.
KEY RESPONSIBILITIES:
- Conduct clinical assessments in community settings to evaluate safety and readiness for Medication Assisted Treatment for substance use and co-occurring disorders.
- Perform comprehensive assessments and LOCATDR determinations to identify the most suitable level of care for substance use disorders and facilitate appropriate referrals.
- Collaborate with clients to develop and update treatment plans that reflect individual preferences and promote realistic behavioral health expectations.
- Employ Evidence-Based Practices, standardized screening tools, and clinical judgment to assess risks related to behavioral health concerns and deliver mobile clinical services in clients' homes and communities.
- Maintain accurate and timely clinical case records, including treatment plans, progress notes, and coordination of care activities, ensuring compliance with agency, funding, licensing, and billing requirements.
- Regularly evaluate treatment progress and enhance client understanding of resources for discharge planning and termination processes.
- Implement crisis intervention and stabilization services as necessary to prevent or manage deterioration and ensure safe community living.
- Assist clients in recognizing the importance of adhering to fee and insurance responsibilities while addressing psychological and financial barriers to payment.
- Utilize supervision to enhance practice skills, improve client services, and uphold accountability to the organization.
- Supervise graduate and undergraduate students when assigned, balancing the needs of clients, the organization, and the educational objectives of the students.
- Engage in data collection and reporting as required.
- Act as a liaison between the organization and the community, representing the agency and promoting its programs at various events and forums.
- Participate in clinical team meetings and support fellow team members, including peers.
- Complete required training sessions as mandated by the organization or governing bodies.
- Facilitate the sharing of data with relevant individuals to support collaborative decision-making and keep clients informed of their progress.
- Perform additional support and follow-up functions aligned with the role and scope of a Social Worker in accordance with organizational standards.
- Identify and implement quality improvement initiatives as part of professional development goals.
- Complete Quality Improvement Training during onboarding and participate in refresher training as needed.
- Foster a culture of inclusion that respects the diverse strengths, perspectives, and experiences of all employees, driving better decision-making and innovation.
- Adhere to all safety protocols and procedures to maintain a safe working environment for all staff, demonstrating a commitment to established safety guidelines.
- Other responsibilities as assigned.
