Key Responsibilities:
- Establish and maintain consistent communication with students and their families to ensure a supportive network.
- Track and report on student attendance, behavior, and academic performance to relevant stakeholders.
- Conduct thorough assessments of individual client histories and requirements to tailor appropriate services.
- Develop and regularly update personalized care plans for each youth.
- Facilitate engaging group and individual activities aimed at skill development and community involvement.
- Coordinate and oversee community service projects and learning opportunities on a regular basis.
- Provide guidance and mentorship to participants in seasonal employment initiatives.
Work Environment: This position operates in a hybrid model, combining in-school engagement with community outreach efforts.
Work Schedule: Standard working hours are Monday through Friday, with occasional evening commitments, ensuring year-round support.
