Clinical Operations Manager

Hackensack Meridian Health is seeking a highly skilled Care Management Supervisor to join our team. As a key member of our care management team, you will be responsible for overseeing the coordination of patient care across the continuum, ensuring timely and efficient care delivery and reimbursement.
Key Responsibilities- Collaborative Care Management: Facilitate collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
- Staff Supervision: Supervise staff with all care management processes, including length of stay, throughput, patient flow, and denials and appeals follow-up.
- Process Improvement: Apply process improvement methodologies in evaluating team members' documentation and ensure that multidisciplinary rounds and team huddles are occurring.
- Leadership and Direction: Provide direction to the multidisciplinary team as needed in difficult cases and obtain, interpret, and present metrics related to care management.
- Performance Improvement: Develop the performance improvement plan of the care manager, document performance, and provide performance feedback, evaluate the work of the team member, and provide reward and recognition for proper and efficient performance.
- Regulatory Compliance: Participate in departmental preparation for regulatory visits and compliance audits and coordinate/facilitate patient care progression throughout the continuum by working collaboratively with the multidisciplinary team.
- Communication and Collaboration: Collaborate with ancillary departments to ensure accuracy of patient demographic and insurance information and assist in the collection and reporting of indicators tracking efficiency of case management processes.
- Data-Driven Decision Making: Use data to drive decisions and plan/implement performance improvement strategies related to assigned staff, including fiscal, clinical, and patient satisfaction data.
- Leadership and Supervision: Collaborate with Physician Advisors/Hospitalist in needs related to Case Management and difficult cases, maintain annual competencies, and ensure training and continuing education of the team in applicable platforms.
- Additional Responsibilities: Assume responsibility for supervision of other case management care coordination managers in the absence of a Supervisor and perform other duties and/or projects as assigned.
- Education: Bachelor's degree, Nursing or Master's Degree in Social Work.
- Experience: At least 3 years full-time experience in an acute care setting.
- Skills and Abilities: Familiar with hospital resources, community resources, and/or resource/utilization management, care coordination, case management, or discharge planning experience, effective decision-making/problem-solving skills, demonstration of creativity in problem-solving, and influential leadership skills.
- Licenses and Certifications: NJ State Professional Registered Nurse License or NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
