Adventist Health Sierra Vista, a prominent healthcare provider since 1959, is dedicated to delivering exceptional care to the community. Our facility features 162 beds, including a Level III Neonatal Intensive Care Unit and a designated trauma center, ensuring comprehensive healthcare services.

Role Summary:

The Patient Safety Change Facilitator is accountable for overseeing the Patient Safety Program. This role involves assisting in the formulation, execution, and assessment of the hospital's quality and patient safety strategies. The facilitator acts as a champion and mentor for patient safety, engaging with staff, leadership, and patients to create initiatives that minimize patient risk. The position promotes systems that encourage a proactive culture of patient safety.

Key Responsibilities:

  • Support leadership in identifying key performance indicators and developing evaluation tools for quality enhancement; coordinate patient safety culture assessments.
  • Devise improvement strategies that incorporate leading practice research and disseminate implementation outcomes across the organization.
  • Collaborate with Infection Control, Risk Management, Medical Staff, nursing leadership, and clinical teams to integrate patient safety and foster a 'Culture of Safety' throughout the organization.
  • Facilitate departmental and Patient Safety Champion meetings, analyze performance metrics trends, identify improvement opportunities, develop action plans, and measure the success of these plans.
  • Lead the Culture of Safety survey, staff debriefings, action plan development, and correlation processes in partnership with the Patient Safety Officer.
  • Engage in multi-facility and system-wide projects, initiatives, and collaborative efforts to enhance patient safety.
  • Coach others in developing patient safety improvement capabilities, focusing on process redesign and measurement.
  • Guide department leadership in planning a systematic approach to continuous patient safety enhancement.
  • Aggregate and analyze data to support Peer Review and Performance Improvement Teams as necessary.
  • Provide oversight and leadership for patient safety activities and deliver ongoing education to staff and medical personnel regarding patient safety principles.
  • Perform additional job-related duties as assigned.

Qualifications:

  • Associate's/Technical Degree or equivalent combination of education and experience: Required
  • Bachelor's Degree: Preferred
  • Three years of relevant technical experience: Preferred
