Location Overview:

Adventist Health Sierra Vista, a prominent healthcare provider, has been dedicated to serving the community since 1959. This 162-bed acute care facility features a Level III Neonatal Intensive Care Unit and is recognized as a county-designated trauma center.

Role Summary:

The Patient Safety Change Facilitator is responsible for overseeing the Patient Safety Program. This role involves assisting in the creation, execution, and assessment of the hospital's quality and patient safety initiatives. The facilitator acts as a mentor and advocate for patient safety, engaging with employees, leadership, and patients to develop programs aimed at minimizing patient risk. The position promotes systems that nurture a proactive culture of patient safety.

Key Responsibilities:

  • Support leadership in defining key performance indicators and creating tools for quality improvement assessment.
  • Coordinate patient safety culture assessments and develop strategies for improvement based on leading practice research.
  • Collaborate with Infection Control, Risk Management, Medical Staff, nursing leadership, and clinical staff to integrate patient safety and the 'Culture of Safety' across the organization.
  • Facilitate department meetings and Patient Safety Champion gatherings, analyze performance metrics, and identify opportunities for enhancement.
  • Lead the Culture of Safety survey and collaborate with the Patient Safety Officer on action plan development.
  • Participate in multi-facility projects and initiatives aimed at improving patient safety.
  • Coach staff in developing patient safety improvement capabilities with a focus on process redesign and measurement.
  • Provide guidance to department leadership in planning a systematic approach to continuous patient safety enhancement.
  • Aggregate and analyze data to support Peer Review and Performance Improvement Teams as necessary.
  • Deliver ongoing education to employees and medical staff regarding patient safety principles.
  • Perform additional job-related duties as assigned.

Qualifications:

  • Associate's/Technical Degree or equivalent combination of education and experience: Required
  • Bachelor's Degree: Preferred
  • Three years of relevant technical experience: Preferred
