Job Title: Home Health Quality Improvement Coordinator

At The Ark of DC, we are seeking a highly skilled and dedicated Home Health Quality Improvement Coordinator to join our team. As a key member of our quality improvement team, you will be responsible for developing, implementing, and evaluating continuous quality improvement programs and activities to ensure the highest level of patient care and satisfaction.

Key Responsibilities:

  • Develop and implement quality improvement initiatives to improve patient outcomes and satisfaction
  • Conduct regular audits and assessments to identify areas for improvement
  • Collaborate with healthcare professionals to develop and implement care plans
  • Monitor and evaluate the effectiveness of quality improvement initiatives
  • Provide education and training to staff on quality improvement best practices

Requirements:

  • Registered Nurse or Licensed Practical/Vocational Nurse
  • Minimum of 2 years home care experience
  • Experience in developing, implementing, and evaluating continuous quality improvement programs and activities
