Emergency Services Case Manager

Equinox is a non-profit human services organization in the Capital Region, providing innovative and effective programs that truly change lives.
The Emergency Services Case Manager provides crisis intervention, short-term case management, and follow-up to victims of domestic violence. Services include supportive listening, initial eligibility assessments, safety planning, information and referral, emergency shelter placement, advocacy, case management, follow-up, and support.
The successful candidate will collaborate with emergency case managers and coordinate with domestic violence staff to ensure clients receive comprehensive services.
Key Responsibilities:- Answer the 24-hour hotline, crisis chat line, and meet with walk-ins to provide crisis intervention and supportive listening.
- Develop safety plans with clients and conduct eligibility assessments for all domestic violence services.
- Assess eligibility and options for victims seeking domestic violence shelter and conduct Equinox shelter intake when appropriate.
- Assist victims with securing alternate emergency housing and provide transportation and food when Equinox shelter is not available.
- Follow up on client needs and provide short-term case management and follow-up to victims who are alternately placed, have left shelter, or have been referred by an Equinox DV team member.
- Provide community education and outreach related to domestic and family violence, elder abuse, and human trafficking.
- Accompany clients to DSS, court, law enforcement, and other agencies as needed, advocating for clients both in person and through phone and written contact.
- Bachelor's Degree in human services or relevant major (2 years of relevant experience with a High School Diploma or equivalent will be substituted in lieu of an advanced degree).
- Minimum one year of human services experience.
- Must have a clean, valid NYS Driver License, with reliable transportation.
