Job Title: Case Manager, Mobile Crisis Team

We are seeking a highly skilled and compassionate Case Manager to join our Mobile Crisis Team. As a key member of our team, you will provide critical support to individuals and families in crisis, working closely with our crisis intervention specialists to develop and implement effective case management plans.

Job Summary

The Case Manager, Mobile Crisis Team will be responsible for providing case management services to clients in crisis, including assessment, planning, and coordination of services. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced, dynamic environment.

Key Responsibilities
  • Assess client needs and develop individualized case management plans
  • Coordinate services with community agencies and treatment providers
  • Monitor and manage client progress, making adjustments to plans as needed
  • Document all client-related communication and interactions in progress notes
  • Collaborate with crisis intervention specialists to ensure effective crisis response and intervention
Requirements

To be successful in this role, you will need:

  • A bachelor's degree in a related field (e.g. social work, psychology, counseling)
  • At least one year of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced, dynamic environment
