Job Title: Case Manager

At Berkshire Farm Center & Services for Youth, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in providing support and guidance to youth and their families.

Job Summary:

The Case Manager will provide initial screening and assessments for all youth admitted to the facility. This will involve conducting individual counseling sessions, identifying and addressing the precipitating difficulties that led to detention, and facilitating communication with the youth's family and other relevant personnel.

Key Responsibilities:
  • Conduct interviews with residents and their families to provide social work support and assistance with adjusting to detention intervention.
  • Consult with other agencies, residents, and families to ensure understanding of current expectations, practices, laws, regulations, and procedures of the Juvenile Justice system.
  • Collect and maintain relevant data to develop individual facility plans for each resident.
  • Record social information to develop case histories.
  • Ensure compliance with all state regulations.
Requirements:
  • Bachelor's Degree in Social Work, Sociology, Psychology, or other human services field is required.
  • Minimum 2-4 years' experience working with juveniles and families.
  • Ability to work a flexible schedule with variable hours, including day, evening, and weekend availability.
  • Knowledge of juvenile justice systems is preferred.
  • Current and valid driver's license with acceptable driving history is required.
  • Ability to work with economically and culturally diverse populations.
Skills:

Excellent verbal and written communication skills are essential for this role.

Salary and Schedule:

The hourly rate for this position is $25.00, commensurate with experience. The schedule is 9AM to 5PM, Sunday through Thursday, with Fridays and Saturdays off.
