Job Title: Case Manager

We are seeking a compassionate and skilled Case Manager to join our team at LifeSpan Resources. As a Case Manager, you will be actively involved in our community, promoting independent living for individuals of all ages.

Job Summary:

The Case Manager will complete all components of on-going case management for waiver and non-waiver programs. This includes establishing and sustaining interpersonal relationships, assisting with Medicaid application processes, and complying with FSSA/IDA manuals and Federal, State, and local laws.

Responsibilities:
  • Establish and sustain interpersonal relationships with clients and community partners.
  • Assist with Medicaid application processes and long-term support planning.
  • Comply with FSSA/IDA manuals and Federal, State, and local laws.
  • Aid in goal setting and program eligibility determination.
  • Execute other assignments as requested.
Qualifications:
  • A minimum of two years of experience in direct service with older adults or individuals with disabilities.
  • A Bachelor's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services.
  • Ability to lift up to 25 lbs. on an occasional basis.
