Job Title: Emergency Services Case Manager

Equinox is a non-profit human services organization in the Capital Region. We are seeking a highly motivated and compassionate Emergency Services Case Manager to join our team.

Job Summary:

The Emergency Services Case Manager provides crisis intervention, short-term case management, and follow-up services to victims of domestic violence. The successful candidate will work closely with our team to ensure clients receive comprehensive services, including supportive listening, initial eligibility assessments, safety planning, information and referral, emergency shelter placement, advocacy, case management, follow-up, and support.

Key Responsibilities:
  • Answer the 24-hour hotline, crisis chat line, and meet with walk-ins to provide crisis intervention, supportive listening, information, and referral.
  • Develop safety plans with clients and conduct eligibility assessments for all Domestic Violence services.
  • Assess eligibility and options for victims seeking domestic violence shelter and conduct Equinox shelter intake when appropriate.
  • Assist victims with securing alternate emergency housing, including transportation and food, when Equinox shelter option is not available.
  • Follow up on client needs and provide short-term case management and follow-up services to victims who are alternately placed, have left shelter, and/or have been referred by an Equinox DV team member for emergency assistance.
  • Provide community education and outreach related to domestic and family violence, elder abuse, and human trafficking.
  • Accompany clients to DSS, court, law enforcement, and other agencies as needed, advocate for clients both in person and through phone and written contact.
  • Participate in coordination and outreach to other community services, including providing services at co-location sites, e.g., DSS.
Requirements:
  • Bachelor's Degree in human services or relevant major (2 years of relevant experience with a High School Diploma or equivalent will be substituted in lieu of an advanced degree).
  • Minimum one year of human services experience.
  • Must have a clean, valid NYS Driver License, with reliable transportation.
Preferred Qualifications:
  • Fluency in Spanish and/or an additional language.
  • Prior experience working with domestic violence, sexual assault, elder abuse, human trafficking, mental health, or substance abuse populations.
  • Prior case management/crisis intervention services experience.
Work Schedule:

Monday through Friday
