Job Overview

Position Title: Case Manager / Intensive Case Manager

Reports To: Program Director

Resources for Human Development is seeking a Case Manager for the Cumberland County Bridge Housing Program (CCBH).

Position Summary:

The Case Manager is responsible for identifying resources essential to address the unique needs of each participant. This role involves ensuring the effective execution of the service treatment plan and advocating for participants to access necessary services. The Case Manager will assess program requirements, identify service deficiencies, and propose actionable strategies. A strong commitment to the values of RHD is expected in all job functions.

Key Responsibilities:

Collaborative Planning:

  • Participate as a member of a multidisciplinary team focused on delivering services from a holistic and values-driven perspective.
  • Contribute to the formulation of recovery plans in partnership with program participants, clinical staff, and other relevant stakeholders.
  • Establish and nurture relationships with community organizations to facilitate participant access to available resources.

Direct Service Delivery:

  • Provide community-oriented services to participants, emphasizing education, resource connection, community engagement, and achievement of recovery objectives.

Administrative Responsibilities:

  • Complete all necessary assessments and documentation as mandated by RHD, funding sources, and regulatory bodies.
  • Perform additional duties as required.

Qualifications:

  • Bachelor's Degree in Human Services or a related discipline.
  • Proficient computer skills.
  • Preferred: One year of experience in Case Management.
  • Required: Two years of general work experience in supporting individuals within behavioral health, addiction recovery, or IDD services.
  • Possession of a valid driver's license.

Work Environment:

Special Working Conditions:

  • Engagement in community-based activities within the service area of the program, including transportation of participants to appointments.
