Position: Registered Nurse Care Coordinator

  • Employ assessment skills to determine individual healthcare needs effectively.
  • Contribute to care management initiatives and diabetes education programs.
  • Exceptional communication and organizational skills are essential for this role.
  • Availability for weekend rotations is required.

Requirements:
  • Valid RN licensure in Maine.
  • AHA BLS certification is mandatory.
  • 3-5 years of experience in acute care, home health, or care management is preferred.
  • Proficiency in computer usage and strong communication abilities are necessary.
Special Requirements:
  • Possession of a Maine driver's license and access to a reliable vehicle.
  • Competence in computer literacy and office equipment is expected.
  • Ability to work collaboratively and foster therapeutic relationships is crucial.
Cognitive Requirements:
  • Emotional maturity and critical thinking skills are essential.
  • Analytical and strategic planning capabilities are required.
  • Adaptability and flexibility in a dynamic healthcare environment are important.
Physical Demands:
  • Capability to stand, sit, and walk for extended durations.

  • Prolonged use of computers and telephones, with good hearing and vision necessary.
