Job Summary:

Nurse On Call is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health Care Specialist. As a key member of our healthcare team, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Responsibilities:

  • Provide quality patient care and perform technical skilled care in the patient's home, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Instruct patients and their families in the care of the patient and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.

Requirements:

  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
