Patient Care Coordinator

Requirements:- Active RN licensure in Maine.
- AHA BLS certification is mandatory.
- 3-5 years of experience in acute care, home health, or care management settings.
- Proficient in computer skills and possess strong communication abilities.
- Possession of a Maine driver's license and access to a reliable vehicle.
- Proficient in computer literacy and use of office equipment.
- Ability to work collaboratively and foster therapeutic relationships.
- Demonstrated emotional maturity and critical thinking skills.
- Strong analytical and strategic planning capabilities.
- Adaptability and flexibility in a dynamic healthcare environment.
- Capability to stand, sit, and walk for extended durations.
- May require lifting up to 20 lbs and traveling to various sites.
- Extended use of computers and telephones, with good hearing and vision required.
- Potential exposure to body fluids and high-pressure situations.
- Frequent interaction with technology and close collaboration with healthcare team members.
