Overview

As part of a High Reliability Organization (HRO), all employees are expected to engage in CHAMP behavior and prioritize patient safety. The pharmacy practice resident is enrolled in a recognized postgraduate training program accredited by the American Society of Health-System Pharmacists (ASHP). This residency program is designed to provide comprehensive training and development in pharmaceutical care.

Responsibilities
  • All patient care duties will be executed in accordance with specific criteria for diverse populations and in compliance with hospital policies and procedures.
  • Medication Management
    Timely review of all medication orders to ensure safe, accurate, and cost-effective drug utilization. This includes:
    1) Performing interventional medication use interventions based on patient-specific data and documenting in appropriate records.
    2) Evaluating drug regimens and recommending modifications when necessary.
  • Adverse Drug Reactions Monitoring
    Preventing, monitoring, detecting, and documenting adverse drug reactions and medication use variances in line with hospital protocols.
  • Formulary Knowledge
    Understanding the formulary process and identifying alternative medications for non-formulary requests, intervening when necessary to address complex therapeutic issues.
  • Clinical Data Interpretation
    Demonstrating knowledge of pharmacokinetics and performing relevant calculations. This includes:
    1) Recommending adjustments to drug therapy based on renal function and laboratory values.
    2) Evaluating the impact of disease states and organ function on drug therapies.
  • Problem Solving
    Effectively communicating and documenting problem identification, findings, recommendations, and follow-up actions. This involves:
    1) Identifying recurring drug-related issues and proposing improvement plans.
    2) Collaborating with the patient care team to resolve patient management challenges.
  • Drug Information Expertise
    Serving as the primary resource for drug information for healthcare professionals. This includes:
    1) Systematically receiving and categorizing drug information requests.
    2) Locating and communicating accurate responses in a timely manner.
  • Interprofessional Relationships
    Understanding the role as a vital member of the healthcare team, demonstrating effective communication skills, and keeping all stakeholders informed of progress and concerns.
  • Leadership and Mentorship
    Overseeing the work of support staff and students, providing guidance and feedback, and demonstrating effective prioritization in patient care.
  • Educational Contributions
    Participating in educational initiatives within the department, including:
    1) Conducting in-services and continuing education for pharmacists and other healthcare professionals.
    2) Engaging in community outreach and awareness programs.
  • Professional Development
    Submitting materials for departmental publications and completing self-assessments for performance evaluations.

Qualifications
Education:
Graduation from an accredited College of Pharmacy with a B.S. or Pharm. D. degree or equivalent experience is required. Eligibility for pharmacist licensure is necessary.

Experience:
Prior experience in a hospital pharmacy setting is preferred.

Special Skills:
Demonstrated knowledge of pharmacology and therapeutics, along with excellent communication skills and the ability to collaborate effectively with others, is essential. Experience in teaching and research is advantageous.

Accountability:
The individual in this role is accountable to the Residency Program Director for maintaining high professional standards and fulfilling the responsibilities of the position.
