- Develop and implement individualized plans of care for each patient, including medical history, physical examination, diagnosis, and treatment.
- Use evidence-based practices and stay up-to-date with the latest medical research and guidelines.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
- Maintain accurate and timely medical records, including documentation of patient interactions, diagnoses, and treatment plans.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay current with changes in medical regulations, laws, and best practices.
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school.
- Completion of an approved internal medicine residency program.
- Current licensure to practice medicine in the State of Connecticut.
- Board certification in internal medicine or a related field.
- Ability to work effectively in a fast-paced, dynamic environment.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with a diverse team of healthcare professionals.
- Experience working in an ambulatory or outpatient setting.
- Bi-lingual in Spanish and English.
- Experience with electronic health records (EHRs) and other healthcare technology.
- Knowledge of quality improvement principles and practices.
