Job Overview:

OBJECTIVE OF POSITION:
To enhance patient care by ensuring the maintenance and documentation of quality care metrics, while systematically reporting findings to identify statistical anomalies.

Efficiently collect and manage information in a manner that is both high quality and cost-effective, aimed at improving the health outcomes of our patients and communities. This role also involves responding to the requirements set forth by oversight agencies (both public and private) while maximizing available incentives and minimizing penalties by achieving the outlined objectives of these organizations.


KEY RESPONSIBILITIES:
1. Establish and uphold quality standards and guidelines essential for achieving recognized benchmarks in primary care. 2. Educate, support, and mentor all team members on the correct documentation processes related to the aforementioned recognitions. 3. Facilitate seamless transitions of care through effective communication with patients, caregivers, clinicians, and facilities, adhering to established policies, guidelines, treatment plans, and standing orders. 4.

Coordinate appointment scheduling in accordance with care standards, utilizing guidelines to identify and address care gaps, while providing support and education on chronic illness self-management as outlined in the physician's treatment plan based on evidence-based practices and population health strategies.

5.

Lead team meetings with physicians and clinical staff to discuss high-risk patients, enhancing the roles of medical assistants and other clinical personnel through team-based services and initiatives aimed at organizing and sustaining optimal patient care quality.


Job Requirements:

EDUCATION:

High school diploma, medical assistant diploma from an accredited vocational institution, or completion of a community college course in medical assisting.


Current medical assistant certification is preferred.
EXPERIENCE:
A minimum of 3 years of recent experience in a medical facility as a medical assistant is preferred, with a focus on primary care office experience.

SKILLS:
Proficiency in Microsoft Office is required. Must possess current health records with the necessary immunizations to work in the healthcare sector (hepatitis B and tuberculosis).

KNOWLEDGE:
Familiarity with healthcare protocols and medical office procedures. Understanding of specific assisting tasks related to various medical practices. Awareness of critical information that must be communicated to patients and their families.

ABILITIES:
Capability to learn and retain information pertinent to patient care procedures. Ability to project a professional and pleasant demeanor. Competence in planning, prioritizing, and executing delegated tasks. Demonstrated compassion and empathy in interactions with others.
