- Accurate Record-Keeping: Maintain accurate and up-to-date patient records, adhering to regulatory requirements and industry standards.
- Clinical Practice Guidelines: Assist in the development and implementation of clinical practice guidelines, ensuring evidence-based care and best practices.
- Collaboration and Communication: Foster strong relationships with patients, caregivers, and medical directors, ensuring open communication and collaboration.
- Nurse Practitioner's License: Possess a valid Nurse Practitioner's license in the state of Pennsylvania.
- Experience and Education: Hold a Master's Degree in Nursing and possess 2-4 years of experience in hospice or a related field, with palliative care experience preferred.
- Valid Driver's License: Possess a valid driver's license and reliable transportation.
- Willingness to Travel: Be willing to travel to patient locations as needed.
