As a Hospice Licensed Practical Nurse (LPN), you will play a vital role in our organization. Key Responsibilities:
• Deliver direct patient care to hospice patients, focusing on effective pain and symptom management
• Implement nursing practices based on thorough assessments and individualized care plans
• Maintain accurate and timely documentation of patient services
• Educate patients and their families/caregivers effectively
• Coordinate hospice services to ensure comprehensive care
• Monitor and evaluate patient responses to treatments and medications
• Engage in professional development and quality assessment initiatives
• Ensure adherence to all applicable state and federal regulations
• Undertake additional responsibilities as assigned

Qualifications:
• Graduate from an accredited practical or vocational nursing program with active licensure in the state of practice
• Minimum of 1 year of nursing experience required; 3+ years preferred
• Experience in community health, hospice, or medical/surgical settings preferred
• Understanding of the hospice care philosophy
• Strong commitment to clinical excellence and documentation accuracy
