Candidate Profile
We are in search of dedicated and empathetic nurse practitioners to conduct assessments in diverse environments such as nursing homes and assisted living facilities. As a member of the OnSite team, you will have the opportunity to cultivate an independent practice while positively influencing the lives of seniors and their families. Our nurse practitioners receive comprehensive administrative support and clinical guidance within a collaborative framework. Key Responsibilities - Primary Care or Mental Health
- Conduct rounds at designated facilities, addressing the acute, subacute, and primary care needs of this vulnerable population.
- Collaborate closely with the OnSite team, nursing home personnel, residents, and their families.
- Proactively monitor patient progress and evaluate the effectiveness of implemented interventions.
- Educate patients and families on best practices for health management.
- Incorporate a variety of treatment modalities, prioritizing non-pharmacological approaches when feasible.
- Possess a Master of Science in Nursing with certifications such as FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP.
- Hold an active nursing license in South Carolina.
- Maintain DEA registration.
- Exhibit excellent communication and documentation skills.
- Demonstrate strong patient assessment capabilities.
- Embrace a proactive, patient-centered work ethic.
