Care Management Director

We are seeking a highly skilled and experienced Care Management Director to join our team at Sutter Health.
Job Summary:The Care Management Director will be responsible for managing the overall operations of the department, ensuring compliance with federal and state regulations, and collaborating with various stakeholders to achieve optimal patient outcomes.
Key Responsibilities:- Lead and manage a team of care management professionals to ensure effective case management and coordination of patient care.
- Develop and implement policies, procedures, and guidelines to ensure compliance with regulatory requirements and organizational standards.
- Collaborate with administrative teams, nurse executives, and directors to ensure seamless communication and coordination of patient care.
- Develop and maintain relationships with community resources, post-acute agencies, and other healthcare providers to ensure optimal patient outcomes.
- Monitor and analyze data to identify trends and areas for improvement, and develop strategies to address these areas.
- Bachelor's degree in Nursing, Health Administration, or related field.
- 8 years of experience in healthcare leadership, with a focus on case management and population health.
- Preferred Master's degree in Nursing, Case Management, or related field.
- CCM or LCSW certification preferred.
