Job Summary:

The Manager, Care Management will be responsible for managing the overall operations of the department, ensuring compliance with current federal and state regulations and guidelines. This role will require frequent contact with various stakeholders, including Administrative Team, Nurse Executives and Directors, Risk Management & Ethics staff, Ancillary Services, Revenue Cycle and Finance Department leaders, Medical Directors, Medical Staff leaders, physicians, contracted providers, community resources post-acute agencies, patients and their families.

Key Responsibilities:
  • Lead innovative case management programs to improve patient outcomes and reduce healthcare costs.
  • Develop and implement effective utilization management and discharge planning strategies.
  • Collaborate with cross-functional teams to ensure seamless coordination of care.
  • Provide coaching, counseling, and administrative support to staff members.
  • Ensure compliance with regulatory requirements and guidelines.
Requirements:
  • Bachelor's degree in Nursing or Health Administration or equivalent education/experience.
  • Preferred Master's degree in Nursing, Case management, or related field; or Masters of Social Work for Social Work Candidates Only or equivalent education/experience.
  • 8 years of experience leading innovative case management programs, utilization management, and discharge planning.
  • 8 years of leadership experience in acute case management, clinical assessment, and coordination of UM Committees/Clinical Review teams.
  • 8 years of experience coaching, counseling, and administering corrective action.
