POSITION TITLE: OCI Peer Assistant Case Manager
FLSA CLASSIFICATION: Non-Exempt
REPORTS TO: Senior Director of Adults and Children's Services, Coordinator of Operation Community Integration
POSITION SUMMARY: The Peer Assistant Case Manager plays a crucial role in delivering Community Psychiatric Support & Treatment and Peer Support services to adult clients enrolled in Intensive Case Management and Community Integration programs. This position focuses on empowering clients to achieve their personal recovery goals while ensuring they remain integrated within the community at the least restrictive level.
KEY RESPONSIBILITIES:
- Deliver services to meet the required productivity standard of five client contacts daily.
- Ensure timely and accurate completion of reports, electronic mileage logs, and timesheets.
- Provide peer support services aligned with the individual treatment plans of clients.
- Maintain precise documentation of service delivery in accordance with MHA, MCO/Medicaid, and COMCARE standards, including treatment plans and progress notes.
- Assist clients in articulating, setting, and achieving their recovery goals as outlined in their treatment plans.
- Facilitate information sharing in both group and individual settings to support the recovery journey.
- Guide clients in effective communication, advocacy, and informed decision-making regarding their treatment and life choices.
- Model positive coping strategies and self-help techniques.
- Refer clients to additional services as needed based on individual requirements.
- Collaborate with other care providers to coordinate services tailored to the clients' needs and functioning levels.
- Ensure consistent medical coverage for clients and assist in necessary reviews.
- Support fellow case managers within the OCI team to maintain continuity of care.
- Engage in outreach efforts to reconnect clients with services and support their recovery objectives.
- Work closely with the OCI Coordinator to ensure comprehensive care and optimal outcomes.
ADDITIONAL REQUIREMENTS:
- Maintain a satisfactory attendance record, including participation in staff meetings and training sessions.
- Demonstrate a solid understanding of technical knowledge relevant to the role.
- Produce a volume of work that meets job expectations.
- Collaborate effectively with team members, sharing essential information and fostering a respectful environment.
- Communicate professionally and effectively with peers, clients, and supervisors, demonstrating active listening and receptiveness to feedback.
- Adapt to new assignments and procedural changes, taking initiative to complete tasks without reminders.
- Execute responsibilities with a high degree of accuracy and conscientiousness.
QUALIFICATIONS: Candidates must possess a Bachelor's degree or equivalent experience in the human services field, with one year of experience substituting for one year of education. Certification as a Peer Specialist is required or must be obtained within the first six months of employment. Applicants should self-identify as current or former primary consumers of mental health services. Proficiency in computer skills is necessary, and preferred qualifications include knowledge of community resources and experience working with adults with mental health challenges. A valid driver's license and access to a personal vehicle are required.
