As a Peer Assistant Case Manager, you will play a vital role in providing Community Psychiatric Support & Treatment and Peer Support services to adult consumers enrolled in Intensive Case Management, Community Integration.
Key Responsibilities:- Provide services to maintain required productivity/billing standard of 5 contacts a day.
- Meet deadlines and ensure accuracy of various reports/paperwork, electronic mileage entries, and electronic timesheets.
- Provide peer support (PSI) services in accordance with the consumer's treatment plan goals.
- Maintain accurate and timely documentation of service provision per MHA, MCO/Medicaid, and COMCARE guidelines and contracts.
- Assist consumers in communicating, setting, and achieving goals and objectives for recovery identified on their individual treatment plan.
- Assist consumers in obtaining or sharing information in a group or individual (one-on-one) setting to aid in the recovery process.
- Model effective coping skills and self-help strategies.
- Make referrals to ancillary services as appropriate per consumer need.
- Collaborate with care providers to coordinate services required to meet identified goals based on the needs of the consumers and congruent with the consumer's level of functioning.
- Maintain acceptable overall attendance record, to include department staff meetings, agency meetings, and training as required.
- Exhibit appropriate level of technical knowledge for the position.
- Produce quantity of work necessary to meet job requirements.
- Work well with a team, keeps others informed of information needed.
- Maintain effective and professional verbal and written interactions with peers, customers, supervisors, and other staff.
