POSITION TITLE: OCI Peer Assistant Case Manager

FLSA CLASSIFICATION: Non-Exempt

REPORTS TO: Senior Director of Adults and Children's Services, Coordinator of Operation Community Integration

POSITION SUMMARY: The Peer Assistant Case Manager plays a crucial role in delivering Community Psychiatric Support & Treatment along with Peer Support services to adult clients engaged in Intensive Case Management and Community Integration programs. This position emphasizes collaboration with clients and various service providers to help individuals achieve their personal goals, ultimately aiming to sustain their community presence at the least restrictive level. The Peer Assistant Case Manager utilizes their own recovery experiences to empower clients in developing essential recovery skills.

KEY RESPONSIBILITIES:

  1. Deliver services to meet the productivity standard of 5 client contacts daily.
  2. Ensure timely and accurate completion of reports, electronic mileage entries, and timesheets.
  3. Provide peer support services aligned with the treatment plan goals of clients.
  4. Maintain precise documentation of service delivery in accordance with MHA, MCO/Medicaid, and COMCARE standards. This includes crafting individualized treatment plans, strengths assessments, and progress notes that reflect appropriate interventions and client progress.
  5. Assist clients in articulating, setting, and reaching their recovery goals as outlined in their treatment plans.
  6. Facilitate information sharing in both group and individual settings to support the recovery journey. Provide coverage for Peer Support Groups as needed.
  7. Educate and guide clients in making informed decisions regarding medications, diagnoses, treatment options, housing, employment, and education.
  8. Demonstrate effective coping strategies and self-help techniques.
  9. Refer clients to additional services based on their specific needs.
  10. Collaborate with care providers to ensure coordinated services that align with client goals and functioning levels.
  11. Assist in securing consistent medical coverage for clients and support the completion of necessary reviews.
  12. Support fellow case managers within the OCI team to ensure seamless client care, including providing coverage during absences.
  13. Engage in proactive outreach efforts to reconnect clients with services and support their recovery objectives.
  14. Work closely with the OCI Coordinator to ensure comprehensive care and positive outcomes.

ADDITIONAL REQUIREMENTS:

  1. Maintain a satisfactory attendance record, including participation in staff meetings and training sessions.
  2. Demonstrate a solid understanding of technical knowledge relevant to the role.
  3. Produce a sufficient volume of work to meet job expectations.
  4. Collaborate effectively with team members, ensuring clear communication and mutual respect.
  5. Exhibit professionalism in verbal and written communications with peers, clients, and supervisors.
  6. Adapt to new tasks and changes in procedures with a proactive approach.
  7. Complete assigned tasks punctually and accurately without the need for reminders.
  8. Execute responsibilities with a high degree of precision.

QUALIFICATIONS: Candidates must possess a Bachelor's degree or equivalent experience in the human services field, with one year of experience substituting for one year of education. Certification as a Peer Specialist is required or candidates must meet the criteria to obtain certification within the first six months of employment. Applicants should self-identify as current or former primary consumers of mental health services. Proficiency in computer skills is necessary. Preferred qualifications include familiarity with community resources, experience working with adults with mental health challenges, strong communication skills, and the ability to build rapport with clients, families, and community providers. A valid driver's license and access to a personal vehicle are mandatory.
