Job Title: Peer Assistant Case Manager

Join our team of dedicated professionals at the Mental Health Association of South Central Kansas as a Peer Assistant Case Manager. In this role, you will provide Community Psychiatric Support & Treatment and Peer Support services to adult consumers enrolled in Intensive Case Management, Community Integration.

Key Responsibilities:
  • Provide services to maintain required productivity/billing standard of 5 contacts a day.
  • Meet deadlines and ensure accuracy of various reports/paperwork, electronic mileage entries, and electronic timesheets.
  • Provide peer support (PSI) services in accordance with the consumer's treatment plan goals.
  • Maintain accurate and timely documentation of service provision per MHA, MCO/Medicaid, and COMCARE guidelines and contracts.
  • Assist consumers in communicating, setting, and achieving goals and objectives for recovery identified on their individual treatment plan.
  • Assist consumers in obtaining or sharing information in a group or individual (one-on-one) setting to aid in the recovery process.
  • Facilitate Peer Support Groups on a fill-in basis to meet departmental needs.
  • Assist and teach consumers to communicate, advocate, and make informed choices in all areas of their lives.
  • Model effective coping skills and self-help strategies.
  • Make referrals to ancillary services as appropriate per consumer need.
  • Collaborate with care providers to coordinate services required to meet identified goals based on the needs of the consumers and congruent with the consumer's level of functioning.
  • Ensure consistent medical coverage for consumer & assist in completion of reviews as appropriate.
  • Assist other case managers on OCI team as needed to ensure continuity of consumer care.
Requirements:
  • Maintain acceptable overall attendance record, to include department staff meetings, agency meetings, and training as required.
  • Exhibit appropriate level of technical knowledge for the position.
  • Produce quantity of work necessary to meet job requirements.
  • Work well with a team, keeps others informed of information needed.
  • Maintain effective and professional verbal and written interactions with peers, customers, supervisors and other staff.
  • Demonstrate the ability and willingness to handle new assignments, changes in procedures, and business requirements.
  • Complete assigned work, meets deadlines without reminders/follow-up from supervisor or others.
Position Requirements:
  • Applicants must possess a Bachelor's degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.
  • The applicant must also be a Certified Peer Specialist or be able to meet requirements to become a Certified Peer Specialist within the first six months of employment.
  • The position requires the applicant to self-identify as a present or former primary consumer of mental health services.
  • Computer experience required.
Physical Requirements:
  • Driving (for purposes of community mobility)
  • Data entry, writing/note taking
  • Lifting/carrying up to 30 pounds
  • Bending/Stooping/climbing stairs
  • Typing for extended periods of time
  • Sitting for extended periods of time
