POSITION TITLE: OCI Peer Assistant Case Manager

FLSA CLASSIFICATION: Non-Exempt

REPORTS TO: Senior Director of Adults and Children's Services, Coordinator of Operation Community Integration

POSITION SUMMARY: The Peer Assistant Case Manager plays a vital role in delivering Community Psychiatric Support & Treatment along with Peer Support services to adult clients engaged in Intensive Case Management and Community Integration programs. This position focuses on empowering individuals to achieve their personal recovery goals while ensuring they remain integrated within the community at the least restrictive level. Utilizing personal recovery experiences, the Peer Assistant Case Manager aids clients in developing essential skills for their recovery journey.

KEY RESPONSIBILITIES:

  1. Deliver services to meet the required productivity standard of five client contacts daily.
  2. Ensure timely and accurate completion of various reports, electronic mileage logs, and timesheets.
  3. Provide peer support services aligned with the client's treatment plan objectives.
  4. Maintain precise documentation of service delivery in accordance with MHA, MCO/Medicaid, and COMCARE standards, including individualized treatment plans and progress notes.
  5. Assist clients in articulating, setting, and achieving their recovery goals as outlined in their treatment plans.
  6. Facilitate group and individual sessions to share information that supports the recovery process.
  7. Empower clients to communicate effectively, advocate for themselves, and make informed decisions regarding their treatment, housing, employment, and education.
  8. Model effective coping mechanisms and self-help strategies.
  9. Refer clients to additional services as needed based on individual requirements.
  10. Collaborate with care providers to coordinate services that meet the clients' identified needs.
  11. Ensure consistent medical coverage for clients and assist in necessary reviews.
  12. Support fellow case managers within the OCI team to maintain continuity of care, including providing coverage during absences.
  13. Engage in outreach efforts to reconnect clients with services aimed at achieving their recovery goals.
  14. Work closely with the OCI Coordinator to ensure comprehensive care and positive outcomes.

ADDITIONAL REQUIREMENTS:

  1. Maintain a satisfactory attendance record, including participation in required meetings and training sessions.
  2. Demonstrate a solid understanding of the technical aspects of the role.
  3. Produce a sufficient volume of work to meet job expectations.
  4. Collaborate effectively with team members and communicate necessary information.
  5. Exhibit professionalism in verbal and written communications with peers and supervisors.
  6. Adapt to new assignments and procedural changes with a proactive approach.
  7. Complete tasks independently and meet deadlines without the need for reminders.
  8. Perform duties with a high level of accuracy and attention to detail.

QUALIFICATIONS: Candidates must possess a Bachelor's degree or equivalent experience in the human services field, with one year of experience substituting for one year of education. Certification as a Peer Specialist is required or must be obtained within the first six months of employment. Applicants should self-identify as current or former primary consumers of mental health services. Proficiency in computer skills is necessary. Preferred qualifications include knowledge of community resources, experience working with adults with mental health challenges, and strong communication skills to foster rapport with clients and community providers. A valid driver's license and access to a personal vehicle are required.
