Care Coordinator for Patient Discharges

Liberty Healthcare and Rehabilitation Services is a provider of comprehensive care solutions. We are currently seeking an experienced professional to fill the position of Care Coordinator.

We offer a competitive salary range of $50,000 - $70,000 per year, based on qualifications and experience. The successful candidate will be responsible for coordinating patient discharges in accordance with established policies and state and federal regulations.

Responsibilities include:

  • Social Program Coordination: Ensure that a meaningful social program appropriate to the needs of each patient is met.
  • Patient Admission and Discharge: Assist with admissions of new residents, completion of related paperwork, and acclimation to the new environment.
  • Discharge Planning: Refer residents or families to the appropriate agency or resource if service cannot be met within the facility, and assist with discharge placement and discharge information as needed.

The ideal candidate will possess a four-year degree and one year of experience in the healthcare field, or an equivalent combination of education and experience. Knowledge of local, state, and federal regulations regarding social needs of patients is also essential.
