Job Description: As a Comprehensive Patient Advocate, you will form a crucial part of our team at Cone Health. Your primary responsibility will be to provide comprehensive and coordinated care to patients, working under moderate supervision. This role requires a high level of analytical skill, enabling you to identify patient needs and develop tailored care plans. You will also need to reconcile information and review care plans to optimize patient outcomes. Our ideal candidate has a strong educational background, including an Associate's Degree and 3 years of experience as a Registered Nurse. A Bachelor's Degree is preferred. For this role, we offer a generous compensation package: $80,000 - $105,000 per annum.
