Position Overview:

As a vital member of Sutter Health, the Patient Care Navigator collaborates with patients from diverse backgrounds, including those facing unique health challenges such as oncology, bariatric, spinal trauma, and stroke. This role is dedicated to guiding patients through a comprehensive range of healthcare services while adhering to established policies and protocols.

The navigator identifies and addresses barriers to care, facilitates access to services, and builds strong relationships with healthcare providers. This includes explaining insurance and payment systems, coordinating care among providers, patients, and caregivers, and ensuring a seamless integration of services within Sutter Health and the broader community.

Additionally, the Patient Care Navigator raises awareness about available resources, including Sutter Health support programs, community services, and clinical trials, while assisting patients and their families in meeting various health-related needs.

Qualifications:

Education: Equivalent experience may be accepted in lieu of a degree. A Bachelor's degree in Social Work, Health Education, or a related field is preferred, or graduation from an accredited nursing program.

Licensure: Candidates must possess a valid RN license in California, or be a Licensed Clinical Social Worker (LCSW), Respiratory Care Practitioner (RCP), Physical Therapist (PT), or Occupational Therapist (OT) within one year of hire. Basic Life Support (BLS) certification is also required.

Experience:

A minimum of five years of relevant experience is typically expected.

Skills and Knowledge:

The ideal candidate will demonstrate proficiency in evidence-based nursing practices and possess in-depth knowledge of specific health conditions, treatment options, and community resources. Strong analytical and project management skills are essential, along with the ability to communicate complex clinical information clearly to diverse audiences.

Computer proficiency, particularly in Microsoft Office Suite and electronic health record systems, is required. The ability to work independently and collaboratively within a team, while managing multiple priorities and ensuring patient confidentiality, is crucial.

Days: Monday - Friday
Hours: Full Time, 40 hours per week
