POSITION OVERVIEW
In this role, you will operate under general supervision to conduct comprehensive evaluations of clients, utilizing various assessment tools to determine their needs.
- Make informed triage decisions and develop clinical formulations based on thorough assessments.
- Collaborate with clients to create individualized treatment plans, providing counseling in various formats, including individual, group, and family sessions.
- Work in conjunction with the treatment team and external partners to coordinate care effectively.
- Exhibit a strong understanding of mental health, substance use, and co-occurring disorders, along with the social factors affecting health.
- Document all services provided in compliance with organizational standards.
- Seek regular clinical supervision and additional guidance as needed.
Client Assessments
- Conduct comprehensive bio-psychosocial assessments for clients of varying complexities, ensuring documentation aligns with agency protocols.
- Employ specific assessment tools to evaluate client needs accurately.
- Utilize diverse information sources to inform treatment planning and develop Interpretive Summaries.
- Demonstrate sound clinical judgment in assessments and decision-making processes.
- Assess the need for involuntary evaluations and coordinate necessary actions with licensed professionals.
- Establish and maintain therapeutic relationships with clients and their families.
- Facilitate individual, family, and group therapy sessions utilizing evidence-based practices.
- Show proficiency in addressing mental health and substance use disorders, along with related social determinants.
- Document clinical services promptly and accurately in the electronic health record.
- Effectively utilize Telehealth technologies when appropriate.
- Develop and communicate treatment plans for clients with complex needs, ensuring they are updated as necessary.
- Serve as a resource for staff, sharing knowledge of community and regional resources.
- Assist the treatment team in addressing challenges related to resource allocation for high-risk clients.
- Ensure timely communication with other providers through appropriate channels.
- Manage a caseload effectively, ensuring follow-up and outreach are conducted as per agency guidelines.
- Maintain current licensure and certification.
- Engage actively in clinical supervision and educational opportunities.
- Pursue continuing education to enhance clinical skills.
- Meet the expected clinical service delivery hours.
- Ensure all documentation adheres to agency and regulatory standards.
- Must hold a current license as an LMSW, LAPC, or LAMFT.
- Demonstrated competency in evidence-based treatments for mental health and substance use disorders.
- Proficient in the use of electronic health records.
- Exhibit professionalism, flexibility, and initiative in all interactions.
- Background in residential or outpatient mental health or substance use programs.
- Experience working with diverse populations, including adults, children, and adolescents.
- Experience in facilitating group therapy sessions.
- Outpatient behavioral health clinic setting.
- Standard working hours with potential for evening or weekend shifts based on operational needs.
