Position Summary

Under the guidance of a Registered Nurse, the Practical Nursing Specialist is responsible for delivering essential care and monitoring the health status of clients. This role involves assisting the RN and physician with treatments and effectively communicating with clients and their families. The Specialist will promptly report any changes in client conditions or additional needs to the supervising nurse.

Key Responsibilities

  • Communicates health and nursing requirements to the RN supervisor
  • Coordinates client admissions, discharges, and necessary follow-ups
  • Assists clients during medical appointments as required
  • Provides nursing services as directed by the RN and physician, ensuring care aligns with the Service Plan/Nursing Care Plan
  • Monitors and records vital signs, weights, and wound assessments, identifying any trends that require attention and reporting them to the RN or physician
  • Relays important information and changes in health status to healthcare professionals and the team in a timely manner
  • Ensures the availability of necessary adaptive equipment for clients
  • Manages medication administration, ensuring accuracy and documenting client responses and any side effects
  • Collaborates with pharmacy staff regarding psychotropic medications and conducts medication reviews
  • Orders and monitors medications as prescribed by the physician, ensuring proper disposal of unused medications
  • Verifies that monthly Medication Administration Records (MARs) are accurately maintained for each client
  • Conducts regular checks of the medication cabinet to ensure compliance with administration orders and documentation
  • Reports client illnesses and potential communicable diseases, working with the RN and other healthcare professionals to maintain a safe environment
  • Documents health status and care services accurately and promptly, adhering to company and state regulations
  • Provides guidance and training for non-licensed staff in care provision and reports any changes observed
  • Completes quarterly nursing evaluations of assigned clients and refers any concerns to the physician for further assessment
  • Ensures incidents are documented, and that clients receive necessary medical attention while communicating with the RN and leadership
  • Participates in clinics and on-call duties to provide after-hours access to clinical staff as needed
  • Engages in quality improvement initiatives and attends team meetings to contribute to interdisciplinary discussions
Qualifications

  • Completion of a state-approved nursing training program (e.g., diploma, associate's degree)
  • Must hold a valid Licensed Practical/Vocational Nurse (LPN or LVN) license in good standing
  • CPR certification required
  • Minimum of 6 months of direct nursing experience preferred
  • Experience in supporting individuals with developmental disabilities or related disorders is advantageous
  • Valid driver's license with a satisfactory driving record
  • Proficient in using computer systems for documentation
  • Strong verbal and written communication skills with all levels of personnel
