Position Summary

Under the guidance of a Registered Nurse, the Healthcare Support Nurse plays a vital role in delivering essential care and monitoring the health status of clients. This position involves assisting the RN and medical professionals in treatment procedures and maintaining effective communication with clients and their families. The Healthcare Support Nurse is responsible for reporting any changes in client conditions or additional needs to the supervising nurse.

Key Responsibilities

  • Effectively communicates health and nursing requirements to the RN supervisor.
  • Manages client admissions, discharges, and necessary follow-ups.
  • Assists clients during medical appointments as required.
  • Provides nursing and health services as directed by the RN and physician, delivering direct care according to the Service Plan/Nursing Care Plan.
  • Monitors and records vital signs, weights, and wound measurements, identifying any trends that require attention and reporting them to the RN or physician.
  • Communicates significant health status changes to healthcare professionals and the team promptly.
  • Ensures the availability of necessary adaptive equipment for clients.
  • Administers medications, ensuring compliance with the six rights of medication administration, and documents client responses and any side effects.
  • Collaborates with pharmacy staff regarding psychotropic medications and conducts medication reviews.
  • Orders, reviews, and manages medications as prescribed by the physician, disposing of unused medications in accordance with policy.
  • Ensures accurate transcription of monthly Medication Administration Records (MARs) in each client’s record.
  • Conducts weekly checks of the medication cabinet to confirm proper administration and documentation of medications.
  • Monitors program/service areas regularly to ensure adequate medical and nursing supplies are available.
  • Reports client illnesses and potential communicable diseases, working closely with the RN and other healthcare professionals to maintain a safe environment.
  • Accurately documents health status, care, and services in compliance with company and state regulations.
  • Provides guidance and education to non-licensed staff regarding care and services, reporting any changes as necessary.
  • Completes quarterly nursing evaluations of assigned individuals and refers any significant findings to the physician.
  • Ensures that incidents are documented, and clients receive necessary medical attention while keeping the RN and operations leadership informed.
  • Participates in scheduled clinics and on-call duties to ensure after-hours access to clinicians for staff and clients.
  • Engages in quality improvement initiatives and attends team meetings to enhance service delivery.
Qualifications

  • Completion of a state-approved nursing training program (e.g., diploma, associate's degree, etc.).
  • Must hold a valid Licensed Practical/Vocational Nurse (LPN or LVN) license in good standing.
  • CPR certification is required.
  • A minimum of 6 months of direct nursing experience is preferred.
  • Experience working with individuals with developmental disabilities or related disorders is advantageous.
  • A valid driver's license with a satisfactory driving record is necessary.
  • Proficiency in using computer systems for documentation is essential.
  • Strong verbal and written communication skills are required to interact with all levels of personnel.
