Job Overview

Position: Hospice Consultant

Location: Southern Middle Tennessee

Schedule: Full-time, Monday to Friday, 8 a.m. - 5 p.m.

Role Summary: The Hospice Consultant plays a crucial role in enhancing referral and admission rates for patients eligible for hospice care within a designated portfolio.

Key Responsibilities:

  1. Collaborate with clinical leadership to identify and cultivate potential referral sources, fostering educational outreach and maintaining professional relationships with existing providers to promote hospice services.
  2. Deliver exceptional customer service and demonstrate critical thinking in all interactions.
  3. Manage a designated portfolio by conducting daily outreach to referral sources.
  4. Maintain accurate documentation of daily activities in the CRM system.
  5. Work alongside referral sources and clinical liaisons to identify suitable placements for patients.
  6. Conduct strategic assessments of territories in partnership with the Director of Performance Improvement, developing and adjusting marketing plans to achieve targeted outcomes.
  7. Track referral sources and assess the utilization of hospice services, preparing reports on relationship activities.
  8. Work independently with minimal supervision.
  9. Organize educational presentations and in-services for referral sources.
  10. Demonstrate effective resource management while considering budget implications.
  11. Maintain a log of referral source expenditures.
  12. Engage with referral sources and families to provide information about hospice care and connect them with clinical resources as needed.
  13. Perform additional duties as assigned.

Supervisory Responsibilities: This position does not include administrative supervisory duties.

Language Proficiency: Ability to produce clear documentation and presentations that accurately reflect the scope of care provided. Proficient in professional correspondence with various healthcare entities and capable of effectively communicating with patients, families, and healthcare professionals.

Mathematical Skills: Competence in basic statistical analysis and the ability to work with fractions, percentages, and ratios.

Reasoning Skills: Strong critical thinking abilities, capable of formulating hypotheses and problem-solving.

Qualifications:

Strong communication and relationship-building skills, with a commitment to patient confidentiality. Understanding and support of agency policies and philosophy. A dedication to ongoing education and quality improvement initiatives.

Certificates and Licenses: Valid driver's license and automobile insurance as mandated by state law.

Physical Requirements: The role requires the ability to sit, talk, hear, and occasionally lift up to 25 pounds. Reasonable accommodations may be made for individuals with disabilities.

Work Environment: Employees may be exposed to moderate noise levels and are often required to use an automobile for work-related tasks.

Education and Experience: A bachelor's degree or equivalent experience, with a preference for candidates with five years of experience in healthcare, hospice sales, and marketing.
