Position: Hospice Consultant

Location: Franklin/Southern Middle Tennessee area

Schedule: Monday - Friday, 8 a.m. - 5 p.m. (40 hours/week)

POSITION SUMMARY

The Hospice Consultant plays a crucial role in driving referral and admission growth for patients eligible for hospice care within a designated portfolio.

KEY RESPONSIBILITIES

  1. Collaborate with clinical leadership to identify and cultivate potential referral sources, providing education and outreach while fostering professional relationships with existing providers to enhance the understanding and utilization of hospice services.
  2. Exhibit exceptional customer service and critical thinking skills consistently.
  3. Manage the designated portfolio by conducting a required number of outreach calls to referral sources daily.
  4. Document daily activities in the CRM system to maintain accurate records.
  5. Work alongside referral sources and hospice clinical liaisons to facilitate patient placements.
  6. Conduct ongoing strategic assessments of accounts in partnership with the Director of Performance Improvement, developing and implementing a comprehensive marketing strategy for assigned territories.
  7. Track referral sources and assess the utilization of hospice services, preparing relationship activity reports.
  8. Demonstrate the ability to work independently with minimal supervision.
  9. Coordinate educational presentations and in-service training sessions for referral sources.
  10. Exhibit effective resource management, understanding the financial implications related to the agency's budget.
  11. Maintain a log of referral source expenditures.
  12. Engage with referral sources and patients/families to provide essential information about hospice care and connect them with clinical resources as necessary.
  13. Perform additional duties as assigned.

SUPERVISORY ROLE

This position does not include administrative supervisory responsibilities over agency staff.

LANGUAGE COMPETENCIES

Ability to produce documentation and presentations that accurately reflect the scope of care provided. Capable of drafting policies and professional correspondence for various healthcare entities. Proficient in effectively communicating, educating, and building rapport with patients, families, healthcare professionals, and community members.

MATHEMATICAL ABILITIES

Ability to engage with mathematical concepts such as basic statistical inference and algebra, applying fractions, percentages, and ratios as needed for data analysis.

REASONING SKILLS

Demonstrate critical thinking abilities, capable of formulating hypotheses and employing judgment to solve problems.

CERTIFICATIONS AND LICENSES

Must possess a valid driver's license and automobile insurance as mandated by state law.

PHYSICAL REQUIREMENTS

The physical demands of this role require the ability to sit, talk, and hear regularly. Frequent use of hands for handling and reaching is necessary. The role may involve standing and occasionally lifting/moving items up to 25 pounds, as well as walking long distances while carrying items. Specific vision abilities required include close vision and the ability to adjust focus.

WORK ENVIRONMENT

While performing job duties, the employee may be exposed to automobile use. The typical noise level in the work environment is moderate.

QUALIFICATIONS

CONTINUING EDUCATION REQUIREMENT

The agency mandates completion of 15 hours of continuing education annually, covering topics that enhance job performance. Regulatory agencies may require additional hours for specific disciplines.

EDUCATION AND EXPERIENCE

A Bachelor's degree or equivalent from a four-year institution is required, along with five years of experience in healthcare, hospice sales, and marketing, or a comparable combination of education and experience.
