Position Overview

We are currently seeking a full-time Social Worker to deliver clinical support to youth and families through our Children and Family Treatment and Support Services (CFTSS) initiative. This program utilizes a collaborative approach aimed at providing personalized and adaptable services by engaging with children in their homes and other community environments. CFTSS focuses on children aged 5-21, emphasizing prevention and early intervention through a diverse array of services that allow for timely support in children's lives.

Key Responsibilities

  • Provide community-based mental health treatment to a caseload of 20 children/youth and their families.
  • Deliver services aligned with the core values of CFTSS: Family-Focused, Community-Based, Multi-Systemic, Culturally Competent, and Least-Restrictive.
  • Implement evidence-based interventions tailored to the unique needs of each child and family.
  • Develop treatment plans and execute services based on assessments and family input.
  • Maintain comprehensive documentation in compliance with CFTSS standards using electronic medical records, including notes, assessments, and treatment plans.
  • Collaborate with other CFTSS and Care Coordination providers through regular communication.
  • Conduct crisis interventions as necessary.
  • Participate in team meetings and contribute to collaborative efforts.
  • Perform additional duties as assigned.

Qualifications

A Master's degree in social work or a related field is required. Candidates must possess an LMSW, LMHC, LCSW, LMFT, or a related licensed clinical degree. Limited permit holders are welcome to apply. A minimum of one year of experience in a behavioral health setting in a clinical capacity is required. Experience in diagnosing using the DSM V and familiarity with behavioral/therapeutic interventions for children, adolescents, and families is essential. Proficiency in computer skills, including Microsoft Office and Electronic Medical Records (EMR), is necessary. Excellent verbal and written communication skills are required. A valid and clean NYS Driver's License is mandatory.
